Multi professional approach to patient care

When the patient is critically ill the nurse must undertake a systematic observation of the patient’s condition and ensure a fast response with the implementation of evidence based nursing interventions.  It is important that all members of the multi professional team are involved in the patient’s care and this requires a coordinated approach to implement effective communication. An evidence based tool used to facilitate timely and effective communication in the multi professional team is the SBAR tool – Situation-Background-Assessment-Recommendation (2008):
